Job description
Collage Home and Community Rehabilitation Partners seeks a Licensed Clinical Social Worker (LCSW) to join the Riverside, CA team as a PRN clinician. You will assess and support medically complex patients in their homes and communities using an integrated, patient-centered approach.
This role requires a Master's in Social Work and LCSW licensure, plus strong planning and communication skills to collaborate with families and healthcare professionals to develop individualized treatment plans.
