Job Summary and Qualifications

Full time (Monday - Friday, days), no nights, no on call, no weekends, no holidays Seeking a Social Worker , Outpatient for our practice who provides clinical expertise to ensure all patients receive high quality, efficient care. What you will do in this role:

Coordinating the provision of patient care services related to such aspects as psychosocial and needs assessment, crisis intervention, and emotional support

Research and advocate for appropriate public assistance resources for clients

Referral to community resources, assistance with socio-economic problems

Communicate with client's care team

Provide crisis intervention (referral to DCF, provider, etc as needed)

Coordinate treatment planning with specialists and outpatient providers as needed Evaluate current patients for appropriate social work services

Judge relative costs/benefits of potential social services and health care actions and choose the most appropriate one.

What Qualifications you will need: EDUCATION: Master of Social Work from CSWE Accredited Institution (Required)

EXPERIENCE: Experience working with clients that are affected by issues including neglect, abuse, domestic violence, substance abuse

Minimum of 2 years prior experience

Inpatient Hospital or Outpatient Mental Health Experience (Preferred)

CERTIFICATION/LICENSE: Licensed Clinical Social Worker (LCSW) in State of Florida (Required) with degree
