Job description
BJC Home Care is seeking an experienced Registered Nurse to coordinate and deliver direct patient care in a hybrid home-health setting. This role requires planning and leading complex care plans, collaborating with an interprofessional team, and ensuring safe patient outcomes across hospital discharge and ambulatory care.
Ideal candidates hold a Nursing Diploma/Associate’s degree (Bachelor’s preferred) with 2–5 years in home health.
