Job description
The Registered Nurse Case Manager (RNCM) at St. Croix Hospice leads each patient’s hospice care team, developing the Plan of Care in collaboration with the Hospice Medical Director, patient, and family.
You will provide day-to-day nursing care, coordinate with physicians and the interdisciplinary team, educate families, ensure HIPAA compliance, and participate in on-call visits as needed.
