Job description
Strive Health in Fairfax, VA is seeking a Clinical Lead to guide a multidisciplinary team in managing CHF, CKD/ESKD and other chronic conditions. You will provide longitudinal, home-based and telehealth care, using analytics and collaboration to reach patients where they are.
You will maintain board certification, manage a defined patient panel, document diagnoses for risk adjustment, participate in rounds, and travel as needed.
