Job description

ChenMed is seeking a Primary Care Physician to provide comprehensive care for older adults, driving outcomes and coordinating with hospitalists, case managers, and care teams. You will lead a care team and engage in transitional care across settings to ensure high-quality, cost-effective care for patients in a value-based model.
