Job description
Polaris Healthcare in Troy, NY seeks a full-time RN in a lead role at our assisted living facility. You will oversee LPNs and home health aides, ensure resident care, and drive continuity of care across the team.
The position requires conducting start-of-care and annual assessments, performing audits, and attending DPS meetings while keeping the director of compliance updated. It is a salaried, benefits-eligible role for 40 hours per week with no weekends or on-call.
