AltaMed Health Services Corporation in Commerce, CA is seeking an experienced RN Case Manager & Care Coordination Team Lead to oversee all PACE participants in ambulatory settings. This role combines clinical leadership with care coordination to ensure high-quality, integrated care for seniors.
Requirements include current CA RN license, 1 year geriatric nursing and managed care experience preferred, and a valid BLS certification.
