Job description
21st Century Home Health Services in California seeks a Registered Nurse Case Manager, Level 2, to lead a small team in planning and delivering home-based care. You will assess patients, coordinate care with an interdisciplinary team, and educate families in their homes.
Responsibilities include supervision of two nurses, developing care plans, performing SOCs and discharges, and ensuring timely documentation. Valid CA RN license, BLS/CPR, and driver’s license are required.
