Job description
Mentor Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.
Licensed Practical Nurse (LPN) - Bilingualism in Spanish required

About the Position
Join our team as an LPN and provide compassionate, person-centered care to adults and seniors in our Adult Day Program. You’ll work closely with participants, families, nurses, and other team members to support their health, independence, and overall well-being.
What You’ll Do
Monitor participants’ health and wellness
Assist with health assessments and nursing evaluations
Administer medications and treatments as ordered
Help implement and follow individual care plans
Provide health education and wellness support
Communicate with nurses, providers, families, and other care team members
Document care and changes in participants’ health
Help maintain a safe, supportive, and caring environment
What You Need
Graduate of an accredited LPN program
Current New Jersey LPN license
Current CPR and First Aid certification
Good communication and organizational skills
Caring, dependable, and compassionate approach to care
Bilingual English/Spanish
