Job description
Live Well Homecare is seeking a Behavioral Health Assessor-Licensed to perform comprehensive face-to-face and tele-mental health assessments, psycho-education, and interventions. You will work under the supervision of the Behavioral Health Manager and collaborate with physicians to determine care levels and safety and discharge plans.
Responsibilities include suicide risk evaluations, treatment planning, referrals, and documentation in EMR.
