Job description
The Rehabilitation Institute of South San Antonio, part of Lifepoint Health, is seeking a Licensed Case Manager to join our inpatient rehab team in San Antonio, TX. This PRN role focuses on care coordination, discharge planning, and seamless communication among patients, families, and the care team.
