Job description
The Rehabilitation Institute of South San Antonio is seeking a licensed Case Manager to coordinate interdisciplinary care, discharge planning, and patient progress within our inpatient rehabilitation setting. You will collaborate with nurses, therapists, and social work to ensure timely care transitions, document care activities, and participate in weekly team conferences.
Qualifications include licensure as RN, Social Worker, or therapy professional, and at least 2 years in inpatient case
