Job description
Health Advocates Network, Inc. is seeking a Licensed Clinical Social Worker to deliver compassionate, evidence-based care across hospital, clinic, and community settings. In this role, you will conduct psychosocial assessments, develop treatment and discharge plans, coordinate community resources, and provide individual and family counseling. You will document in EHR systems, collaborate with interdisciplinary care teams, and help reduce barriers to care.
