Key Responsibilities
Conduct thorough clinical assessments and develop personalized treatment plans for diverse patient populations.
Utilize evidence-based therapeutic interventions to address complex patient issues and facilitate positive change.
Maintain accurate and timely clinical documentation in accordance with organizational standards and compliance requirements.
Work closely with a multidisciplinary treatment team, engaging in regular case reviews and collaborative decision-making to optimize patient care.

Stay current with the latest mental health research, practices, and regulatory guidelines to enhance the quality of care provided.
Patient Population / Client Focus
This position primarily serves a diverse range of patients dealing with acute and chronic mental health conditions within a hospice setting. Your expertise will be crucial in assisting patients and families in the process of grief, transition, and end-of-life care, ensuring they receive both psychological support and practical resources. Your role in fostering resilience and coping strategies is essential in improving their quality of life.
Qualifications
A Master's degree in Social Work (MSW) from an accredited institution is required.
Must possess a valid LCSW license in the state of Arizona.
Demonstrated knowledge and practical experience in solution-based therapy and other therapeutic modalities.
Exceptional communication skills with the ability to establish trust and rapport with patients and colleagues.
Strong problem-solving capabilities, essential for navigating complex patient scenarios.
Preferred Experience
Experience working in hospice or palliative care settings is preferred but not mandatory.

Compensation and Benefits
Competitive salary range of USD 80,000 – 100,000 annually based on experience and qualifications.
