Job description
Outcome HC in Ohio is seeking a Licensed Director of Social Worker (LSW) to lead the Social Services Department and ensure residents receive comprehensive, person-centered care aligned with regulations. You will perform assessments, develop care plans, facilitate therapy-related activities, and coordinate preadmission and discharge planning with interdisciplinary teams.
