Job description
Carle Health seeks a Social Worker to deliver patient-centered, community-based care within an interdisciplinary healthcare team. In this role, you will complete psychosocial assessments, develop individualized care plans, and coordinate resources to address social determinants of health. You will provide crisis intervention, brief counseling, and advocacy to support safe transitions of care and improved health outcomes.
