Licensed LVN/RN MDS Coordinator
Job Summary
We are seeking a highly skilled and compassionate Licensed RN MDS Coordinator to join our team at Avir at Sealy. As a champion of patient-centered care, you will play a vital role in ensuring the highest quality of life for our residents. As a key member of our healthcare team, you will work closely with interdisciplinary professionals to develop and implement effective care plans, promoting a culture of excellence and compassion.
Responsibilities:
Collaborate with interdisciplinary team to develop and implement individualized care plans, ensuring seamless transitions and effective communication among all caregivers
Conduct comprehensive assessments of residents' needs, identifying areas for improvement and proposing recommendations for optimal care delivery
Coordinate and manage MDS (Minimum Data Set) assessments, ensuring accuracy and timeliness of submissions
Maintain accurate and up-to-date resident records, ensuring compliance with regulatory standards and internal policies
Participate in quality improvement initiatives , contributing to the development and implementation of strategies to improve resident outcomes and satisfaction
Provide education and support to staff members, families, and residents, promoting a culture of open communication and compassion
