Job description
Kaiser Permanente is seeking an LMSW Case Manager to provide compassionate, patient-centered care in our Healthcare & Medical Services department. In this role, you will complete psychosocial assessments, develop individualized care plans, coordinate services across interdisciplinary teams, and advocate for patients and families. You will connect patients to community resources, support care transitions, and monitor outcomes to improve health and well-being.
