About the Role
As a Licensed Nurse RAI Coordinator, you will be responsible for:
Conducting and facilitating RAI assessments for residents
Coordinating with healthcare teams to ensure comprehensive assessments
Maintaining accurate and up-to-date records
Identifying and addressing resident needs and concerns
Collaborating with other departments to ensure seamless care delivery
Providing exceptional customer service to residents, families, and healthcare providers
Responsibilities
Conduct and facilitate RAI assessments for residents, ensuring thorough and accurate documentation
Work closely with the interdisciplinary care team to identify and address resident needs and concerns
Develop and maintain accurate and comprehensive resident records
Identify and report potential issues or concerns to the care team
Collaborate with other departments to ensure seamless care delivery and communication
Provide education and support to families and healthcare providers as needed
