About the Job:

Outreach & Education: Champion engagement by raising awareness of mental health services through client-driven wellness initiatives, proactive check-ins, workshops, and educational programs.

Resource Navigation: Identify and address social determinants of health (like access to food or transportation), bridging the gap to necessary community resources to enhance overall well-being.

Team Building: Cultivate strong, collaborative relationships with health center staff and leadership, adapting to evolving center and patient needs.

What You'll Bring to the Team (Requirements): Education: A Master's in Social Work (M.S.W.) from a CSWE-accredited program-or a related behavioral health field.
