Monitor residents for changes in physical condition, behavior, appetite, mobility, or alertness and promptly report concerns to the nurse.

Respond to call lights and resident needs in a timely and appropriate manner.

Follow individualized care plans, infection prevention procedures, fall prevention protocols, and facility safety policies.

Maintain resident privacy, dignity, comfort, and confidentiality at all times.

Keep resident areas clean, organized, and safe.

Report suspected abuse, neglect, injuries, and safety concerns through the appropriate channels.

Schedule and Work Environment

This is a PRN position offering shifts based on facility staffing needs and confirmed availability. Hours and shifts are not guaranteed.

LNAs should be comfortable working in a busy skilled nursing environment, providing hands-on resident care, managing competing priorities, and maintaining professional standards throughout their assignments.

PRN scheduling requires flexibility, reliable attendance, and a commitment to the shifts accepted.

Requirements

Ability to follow care plans, recognize changes in resident condition, and promptly notify licensed nursing staff.

Willingness to complete required onboarding, competency assessments, and applicable background and employment checks.

Ability to meet the essential functions of the position, with reasonable accommodation where applicable.

Preferred Qualifications

Previous experience in a skilled nursing facility, nursing home, rehabilitation center, or long-term care setting.

Experience caring for residents with dementia, limited mobility, incontinence, or increased fall risk.

Familiarity with safe transfers, repositioning, gait belts, and mechanical lifts following appropriate training.

Experience obtaining vital signs and documenting resident care.

Familiarity with electronic health records or electronic care documentation.

Availability for additional PRN shifts, including weekends, when needed.

Ideal Candidate

The ideal candidate is compassionate, dependable, attentive to detail, and committed to resident safety. They demonstrate sound judgment, remain calm under pressure, communicate concerns promptly, and work well with nursing staff.
