Job Description:
Role: Home Health Aide
Rate: $25.00/hour
Main Office Location: Londonderry, NH
Service Area: Nashua, Merrimack, Londonderry and surrounding areas
Schedule: Monday - Friday (8:30-5:00pm)
Job Type: Full Time
Qualifications: Licensed Nursing Assistant (CNA) in New Hampshire preferred ; Home Health Aide certification accepted
Must be 18 years of age or older
Ability to read and follow written care plans and document services provided
Ability to work independently with minimal supervision
Compassion and empathy for individuals who are ill, injured, or elderly
Current CPR certification required
Strong interpersonal skills, patience, and professionalism
Valid driver's license and reliable insured vehicle
Ability to assist patients with repositioning, transferring, and other physical tasks.
Responsibilities: Provide personal care assistance such as bathing, grooming, dressing, and mobility support
Observe and report changes in patients' condition to the supervising nurse or therapist
Document care provided and patient observations accurately
Follow the care plan established by the Registered Nurse
Maintain a safe and supportive environment for patients in their homes
Promote quality, continuity, and safety of patient care
