Job description
428 Healthcare is seeking a Licensed Physical Therapist to join our Assisted Living facilities in Lamar, CO. You will help residents regain independence and improve physical function through focused evaluation and treatment.
The role requires collaboration with a multidisciplinary team, documenting progress, and delivering safe, patient-centered care. A Bachelor’s degree in physical therapy and current Colorado licensure are required.
