Job description
CarePartners is seeking a compassionate Licensed Practical Nurse to deliver skilled, in-home nursing care for patients across our Connecticut service area. The LPN will provide hands-on clinical care, including assessments, medication administration, wound care, and patient education, while supporting patients’ independence and recovery at home. Working within a collaborative, patient-centered team, you’ll coordinate with RNs, therapists, and physicians, document care in the EMR, and uphold high standards of safety and quality. This role offers meaningful relationships with patients and families, ongoing training, and opportunities for professional growth in a supportive environment.
