Job description
VNS Health is seeking a Licensed Practical Nurse to provide compassionate home care services to patients in the Bronx. In this role, you will deliver direct nursing care, administer medications, monitor patient conditions, and support care plans under RN and physician supervision. You will educate patients and families on treatments and self-care, document all services accurately, and collaborate with an interdisciplinary care team.
