Job Description Job Description About the Role: Halcyon Home Care LLC is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team in Alpharetta, GA. Responsibilities: Provide direct patient care including wound care, medication administration, and vital sign monitoring
Develop and follow individualized patient care plans in coordination with RNs and physicians
Observe and document patient conditions, progress, and any changes in health status
Educate patients and family members on care procedures, medications, and health management
Administer IV therapy, catheter care, and other skilled nursing procedures as ordered
Communicate effectively with the interdisciplinary care team to ensure continuity of care
Maintain accurate and timely clinical documentation in compliance with home health regulations
Requirements: Active LPN license in the state of Georgia (required)
Minimum 1 year of nursing experience, preferably in home health or a clinical setting
Strong knowledge of skilled nursing procedures and patient care protocols
Excellent communication, time management, and organizational skills
Ability to work independently and exercise sound clinical judgment
Valid driver's license and reliable transportation for home visits
CPR/BLS certification (required or willingness to obtain)
