Job DescriptionJob DescriptionCounsel Culture Health Institute is seeking a dependable, compassionate, and professional Licensed Practical Nurse (LPN) to support our residential crisis stabilization program. We serve individuals who may be experiencing mental health challenges, emotional crisis, substance use concerns, or a need for short-term stabilization in a safe and structured environment.
This role is a strong fit for an LPN who can balance clinical responsibility with patience, professionalism, and good people skills. We need someone who understands that our clients are not just “patients” on a chart. They are people working through real-life situations, and our job is to help them stabilize with dignity, structure, and care.
Responsibilities
Monitor client health, safety, and overall wellness during assigned shifts
Administer medications according to physician orders and program policy
Document medication administration, client observations, incidents, and shift notes accurately
Communicate client concerns or changes in condition to the RN, clinical team, or supervisor
Support crisis stabilization services in a calm, respectful, and trauma-informed manner
Assist with admissions, discharges, health screenings, and routine client checks
Help maintain a safe, clean, and structured residential environment
Follow all agency policies, confidentiality standards, HIPAA requirements, and state regulations
Participate in staff meetings, trainings, and care coordination as needed
Work as part of a team with residential aides, QMHPs, counselors, and leadership
Qualifications
Current Virginia LPN license in good standing
Experience in behavioral health, residential care, crisis stabilization, substance use, or mental health preferred
New graduates may be considered if they are dependable, coachable, and professional
Strong documentation and communication skills
Ability to stay calm and make sound decisions in crisis situations
Must be reliable, punctual, and able to work as part of a team
Must pass required background checks and meet all state/provider requirements
CPR/First Aid certification preferred or willingness to obtain

The ideal candidate is someone who is clinically responsible, emotionally steady, and able to connect with clients respectfully. You should be able to follow policy, communicate clearly, and bring positive energy to a team environment. We are looking for someone who can be firm when needed, compassionate always, and professional at all times.
Schedule
Day, evening, overnight, and weekend shifts may be available. Schedule will be discussed during the interview process.
Compensation
Pay is based on experience, qualifications, and schedule availability.
