Job description
VNS Health is seeking a dedicated Licensed Practical Nurse to deliver compassionate home and community-based care throughout New York City. The LPN will provide direct patient care, administer medications, monitor health status, and support individualized care plans under the supervision of an RN. In this role, you’ll educate patients and families, document care accurately, and collaborate with an interdisciplinary team.
