Licensed Practical Nurse (LPN) — Primary Care
Pay:
$34.00 - $36.50/hr
Full-Time | 9 AM to 5 PM | Red Hook and Astoria
About
Rockwell Health Center is a multi-site outpatient healthcare network with clinics in Red Hook, Brooklyn and Astoria, Queens. We are seeking an experienced Licensed Practical Nurse (LPN) to support our primary care operations. You will work closely with our clinical team to coordinate patient care, assist with clinical procedures, and make sure patient needs get timely follow-up.
Key Responsibilities
Coordinate durable medical equipment (DME) orders and prior authorizations
Communicate lab results and follow-up instructions to patients, as directed by the treating provider
Administer vaccines and injections per provider orders and clinic protocols
Reinforce patient education on chronic conditions, including diabetes and hypertension, as directed by the clinical team
Assist with medication reconciliation and keep patient medication records accurate
Prepare patient letters and support clinical documentation in our Electronic Health Record (EHR), eClinicalWorks (eCW)
Track patient follow-ups and flag outstanding needs to providers
Room patients, take vital signs, and step in on medical assistant tasks if needed
Work with providers and clinical staff at both sites to keep patient care and clinic flow on track
Perform other nursing duties within the LPN scope of practice, as assigned
Requirements
Active, unrestricted New York State Licensed Practical Nurse (LPN) license and current registration
At least one year of experience as an LPN
Able to manage a steady patient flow while keeping follow-ups and documentation current
Clear, organized communicator with patients, providers, and administrative teams
Available five days per week, 9:00 AM to 5:00 PM, on-site at our Red Hook or Astoria clinic
Preferred skills
Experience in primary care, urgent care, or an emergency department
Spanish proficiency
Current Basic Life Support (BLS) certification
