Location Detail: 680 Main Street Watertown (10326)
Shift Detail: Mon-Fri 8am-4:30pm
Cover Naugatuck/Middlebury/Southbury/Watertown area Requires coverage 1 weekend per month, 1 major and 2 minor holidays per year.

Job Description
Facilitate communication between the patient and the
interdisciplinary team.
Develop and maintain a therapeutic relationship by providing
education and promoting self-advocacy and independence.
Utilize state-of-the-art technology to document skilled
services provided.
Qualifications
Licensed Practical Nurse with Valid CT LPN license
One year of LPN experience; Homecare experience preferred.
Valid driver's license and reliable transportation required.
Computer proficiency required.
