Job description
Lehigh Valley Health Network seeks a Licensed Psychiatric Case Manager to coordinate care for patients with behavioral health needs across our hospitals and clinics. In this role, you’ll complete comprehensive assessments, develop individualized care and safety plans, and connect patients and families with psychiatric, medical, and community resources. You’ll collaborate closely with psychiatrists, therapists, nurses, and social service agencies, monitor progress, support treatment adherence, and maintain thorough documentation—all within LVHN’s compassionate, patient-first, and collaborative culture.
