Responsibilities:
Provide social work services to patients and families, including assessment, treatment planning, and counseling
Develop and implement individualized care plans to address the psychosocial needs of our patients
Collaborate with the healthcare team to ensure comprehensive care and communication
Facilitate group therapy sessions and support groups as needed
Maintain accurate and timely records of patient interactions and care plans
Participate in multidisciplinary team meetings and provide education to the healthcare team on social work issues
Utilize emotional intelligence and respect to provide empathetic and compassionate care
Requirements:
Licensed Social Worker (LSW) in the state of Massachusetts
Master's degree in Social Work (MSW) or related field
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Knowledge of community resources and benefits
