Red Oak Health & Rehab is seeking an experienced and dedicated Licensed Social Worker!
Job Expectations:
Assist in planning, organizing, implementing, evaluating and directing the overall operation of the Social Service Department. The Social Worker will provide medically related social work services so that each resident may attain or maintain the highest practicable level of physical, mental and psychosocial well-being.

Resident/Family/Physician Intervention
Psychosocial Services
Vision, Hearing, Dental Services
Assessments & Care Planning
Discharge Planning
Requirements: Prefer 2 Years Social Work experience in a Long Term Care or Hospital
4 year Degree
Licensed Social Worker in the State of Texas
Education / Qualifications:
• 4 year Degree
• Social Worker License
Knowledge / Experience:
• At least 2 years experience in a Skilled Nursing Facility or Hospital
Licensure / Certifications:
• Social Worker License
