Job description
Navigator Homes of Marthas Vineyard is seeking a Full-time Licensed Social Worker to support residents in our long-term care setting on Martha's Vineyard. The role reports to Alliance Health Executive Director and emphasizes compassionate, resident-centered care.
The successful candidate will have an MA LCSW with LTC experience and strong discharge planning knowledge, along with proficiency in EMR, MDS, and care plan development to enhance quality of life for elderly residents.
