Licensed Social Worker
Job Summary:

Responsibilities:
Develop and implement comprehensive social work assessments, care plans, and interventions to address the emotional, psychological, and social needs of our residents
Provide individual and group counseling services to residents, addressing issues such as grief, adjustment to change, and coping with illness
Facilitate support groups, education programs, and other interventions to foster a sense of community and promote resident well-being
Collaborate with the interdisciplinary team to develop and implement plans of care that address the physical, emotional, and social needs of our residents
Communicate effectively with residents, families, and healthcare professionals to ensure that the emotional and social needs of our residents are met
Maintain accurate and up-to-date records, including assessments, progress notes, and discharge summaries
