Thrive Rehabilitation of Pearland, a premier post-acute rehabilitation and in-patient skilled nursing facility, is seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our night shift team in Pearland, Texas. As an LVN, you will play a crucial role in delivering high-quality patient care, promoting patient recovery, and fostering a positive patient experience. Responsibilities
Provide direct patient care, including administration of medications, wound care, and vital sign monitoring, under the supervision of a registered nurse.
Develop and implement individualized patient care plans in collaboration with interdisciplinary teams.
Participate in shift reports, patient assessments, and quality improvement initiatives to ensure optimal patient outcomes.
Utilize effective communication skills to collaborate with patients, families, and healthcare professionals to address patient needs and concerns.
Maintain accurate and timely records, including patient charts and medication records.
Requirements
Current LVN licensure in the state of Texas.
High school diploma or equivalent required.
