StarCare Specialty Health System (StarCare) is seeking a full-time Transition Support Team Lead to join its outstanding LIDDA team. The Transition Support Team (TST) Lead coordinates the daily operations of the TST program, ensuring effective support for individuals with Intellectual and Developmental Disabilities (IDD) who are at risk of institutionalization or transitioning from institutional settings. Responsibilities include overseeing program implementation, developing and maintaining the HHSC-approved TST Plan, providing training and technical assistance to LIDDAs and providers, facilitating multidisciplinary consultations, monitoring and analyzing program data, and supporting quality service delivery. The TST Lead assists with evaluating service utilization, compliance, productivity, and program outcomes; ensures timely completion of documentation and training requirements; supports special projects and assignments; and maintains collaborative relationships with HHSC, community partners, providers, stakeholders, and Center staff.
Responsibilities:
Coordinate and oversee the daily operations of the Transition Support Team (TST) to support individuals with Intellectual and Developmental Disabilities (IDD) who are at risk of institutionalization or transitioning from institutional settings.
Develop, implement, and maintain the TST Plan in compliance with Health and Human Services Commission (HHSC) requirements.
Provide training, educational opportunities, technical assistance, and consultation to LIDDAs, providers, and service planning teams to promote effective, evidence-based support practices.
Facilitate multidisciplinary consultations, peer reviews, and stakeholder collaboration to enhance service delivery and program outcomes.
Monitor, analyze, and report program data, including service utilization, costs, trends, and performance measures.
Assist the Program Diversion Coordinator in evaluating program effectiveness and ensuring compliance with productivity, documentation, training, and quality standards.
Maintain timely and accurate documentation, support special projects and assignments, and communicate effectively with supervisors, staff, and community partners.
Develop and maintain collaborative relationships with HHSC, state and local agencies, contracted providers, stakeholders, and other key partners.
Maintain regular and consistent attendance for the assigned work hours.
Ensure all required training and credentials are current and compliant with Center, federal, state, and local regulations, in addition to StarCare policies and procedures.
Complete special departmental projects and other responsibilities with similar skill and work conditions as assigned.
Support all activities in alignment with StarCare's mission, values, and operational goals.
Schedule:
This is a full-time, Hybrid, position scheduled Monday - Friday, 8am - 5pm, with flexibility to meet the needs of the department.

Qualifications: Master's degree from an accredited college or university in a Human Services field, such as Psychology, Sociology, Family Studies, or a related discipline required.
Fully licensed mental health professional in the State of Texas LPC, LCSW, or LMFT preferred or LPC Associate, LMFT Associate, or another equivalent licensure required.
Qualified Intellectual Disability Professional (QIDP) as defined by applicable federal regulations.
Minimum of two years of experience supporting individuals with Intellectual and Developmental Disabilities (IDD) with behavioral health needs or coordinating access to mental health services required.
Knowledge of human services delivery systems and services for individuals with IDD.
Must have a current Texas driver's license and be insurable under the Center's insurance.
Must pass all required pre-employment and annual screenings applicable to the position or services provided, as required by law, regulation, or agency policy, which may include criminal background, nursing registry, MVR, and drug screening based on applicable requirements.
