Job description
Enhabit Home Health & Hospice is seeking a licensed Master of Social Work to join our care team in Scottsdale, AZ. This role administers social services to patients, teaching families and the care team according to physician orders and the plan of care.
Requirements include MSW, state licensure, at least one year in health care social work, and basic tech skills. Medicare/home health experience preferred; a driver license is needed.
