Job description
Pharmacist – Inpatient
Join our team as an Inpatient Pharmacist, where you will play a critical role in patient care by ensuring the safe and effective use of medications in a hospital setting. This position is ideal for licensed pharmacists passionate about collaborating with healthcare teams to optimize patient outcomes.
Key Responsibilities:
Dispense medications accurately based on physician orders and ensure proper dosages for hospitalized patients.
Monitor patients' medication therapies to prevent adverse reactions and interactions.
Collaborate with physicians, nurses, and other healthcare professionals to develop and adjust treatment plans.
Provide guidance on the preparation and administration of intravenous (IV) medications.
Educate patients and families about medications upon discharge to ensure proper use at home.
Maintain accurate records of all medications dispensed and ensure compliance with hospital policies and regulations.
Work Environment:
The role is based in hospital inpatient units, requiring close collaboration with multidisciplinary teams in a fast-paced environment.
This position may require working evenings, weekends, or holidays to meet patient care needs.
