Pride Health is actively hiring a Physical Therapist (PT) to join our client's Home Health Team .
Location: Parsippany, NJ 07054
Pay Rate: Up to $62.00/hour on W2
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Weekly Hours: 40 hours
Productivity: 30 points/week
Coverage Area: Morris County
On-Call/Weekend: One weekend per month
Mileage: Reimbursed between patient visits
Patient Setting: Home Health
Key Responsibilities
Provide physical therapy services to patients in their homes.
Evaluate patient needs and develop individualized physical therapy plans of care.
Deliver treatment to improve mobility, function, and independence.
Monitor patient progress and communicate with physicians and the healthcare team.
Complete timely and accurate patient documentation and visit notes.
Meet productivity expectations and participate in the monthly weekend rotation.
Requirements
Active Physical Therapist (PT) license.
Home Health experience required.
CPR certification.
Valid driver's license, auto insurance, and reliable vehicle.
Strong communication, clinical assessment, and care planning skills.
