Productivity expectation: 30 points per week

Approximately 6-7 patient visits per day

Local Pay Package

$58/hr

$2,320/week (based on 40 hrs)

Overview

Provide physical therapy services to patients in a home health setting.

Travel throughout the assigned service area providing evaluations, treatment, and documentation.

Responsibilities include:

Patient evaluations

Treatment planning

Home health therapy visits

Documentation and care coordination

Patient and caregiver education

Keweenaw County

Partial Baraga County

Mileage reimbursed between patient visits

Charting System: Homecare Homebase (HCHB)

Teams interview required

Requirements Active Michigan Physical Therapist License required Graduate of an accredited Physical Therapy program Home Health experience preferred Homecare Homebase (HCHB) experience preferred ❤ BLS/CPR required if applicable Reliable transportation required Comfortable working independently in the field Amergis ID:
