Overview
Provide physical therapy services to residents in a skilled nursing and rehabilitation setting.
Responsibilities include:
Patient evaluations
Individualized treatment plans
Therapeutic interventions
Care coordination
Progress monitoring
Patient and family education
Documentation within NetHealth
Additional Details
Charting System: NetHealth
In-person interview required
Typical treatment time:
30-45 minutes per patient
85-90% productivity expectation
One-day facility orientation followed by on-the-job training
Permanent full-time opportunity
Requirements Active Michigan Physical Therapist License Skilled Nursing Facility (SNF) experience REQUIRED Graduate of an accredited Physical Therapy program ❤ CPR/BLS if required Must be interested in direct hire employment Amergis ID:
