Does your Facility require physicians to be board certified?: Yes
Fellowship: Preferred
Minimum Board Certification: Certified
What is the license requirement to bid?: Licensed in AL or IMLC
Estimated Credentialing Timeframe (Days): 60 days
Must be willing to cover weekend call
Centers (FRI 5pm- MON 7am)
Coverage dates needed: November- 25th, 26th, 27th, and 29th
December- 2nd, 3rd, 4th, 23rd, 24th, 26th, 27th, 30th, 31st
Provider will be credentialed at all 3 sites
It is the expectation for locums to hold TAVR privilege
