Provider Type: Advanced Practice Coverage Type: Scheduled Clinical Hours EMR System: Epic Practice Setting: Outpatient Patients Per Shift: 18 Does your Facility require physicians to be board certified?: Yes Fellowship: NA Minimum Board Certification: Certified Estimated Credentialing Timeframe (Days): 90 What is the license requirement to bid?: Licensed in CA only Chart closure within 72 hours Schedule = 18 PPD, minimum is 18 PPD Monday-Friday 8-5 A mix of peds/adults A decision is typically made within 7-14 days APCs with 3 years of experience in a similar setting PAs must have a CA Controlled Substance Education Course Certificate NPs must have a Furnishing License with Schedules II Active DEA needed before the start Credentialing takes 30-90 days The following immunizations are required for credentialing (before the start date): Flu Vaccine, TB Status/X-Ray Results (results must be within 90 days before the start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years). HCO & Vendor agree to a 30-day notice for ending all assignments.
