Doctor of Medicine | Anesthesiology - General/Other Location: Fortuna, CA Employer: Alumni Healthcare Staffing Pay: Competitive weekly pay (inquire for details) Contract Duration: 2 Weeks Start Date: ASAP About the Position Specialty State CA Location 3300 Renner Dr, Fortuna, CA 95540 Requirements Locums Fortuna, CA CRNA *CLEAN FILES PREFERED* Providers with settlements, DUIs/drug issues, restrictions, terminations, Medical Board actions, etc. are not eligible for temporary privileges The RFA, CV, and 24 months of relevant activity is needed A completed application separate from / following the above step References on standby to respond immediately Rate is higher for this need as well. SCHEDULE • Coverage Type: Inpatient • Schedule Type: Hospital and Call • Call Type: 24hr weekend call (beeper) • Schedule (per week): ongoing shift coverage • Dates of Need: ongoing shift coverage, specific shifts to be identified. • Shift Times: above • Lunch N/A unpaid • Provider Min Shifts Required: 5 shifts a month but could be more/less offered • Guaranteed Hours per Day: 8 • Hours Worked for OT to apply: 8 • Rates to be Filled Out on Bid: Clinic and Call: Regular, Orientation, Overtime, Holiday, Night Call, 24 Hour Call, Call-Back. CREDENTIALING • Current CV Must be uploaded to bid • License: Active • DEA Required: Yes • Boards: BC or truly BE • Skills/Certs: • Other: Privileging Document attached ADDITIONAL DETAILS Please note the following travel deviations from our contract will apply to the assignment (or current confirmation agreement): NOT reimbursed: o Mileage from provider’s home to departing airport, as well as from airport to home upon return. o Ride share (Lyft/Uber/etc.) and/or taxi from provider’s home to departing airport, as well as from airport to home upon return.
