Schedule/Availability Requirements:
April 07 - April 10; 8a - 4:30p (site is aware these dates may not get filled due to compliance timeframes)
April 14 - April 17; 8a - 4:30p (site is aware these dates may not get filled due to compliance timeframes)

FTE: 1
Required Procedures: Provider must be able to do SRS and SBRT.
EMR: EPIC, Mosaiq
Support Staff: One APP will provide coverage
Reason for Coverage: vacation coverage
Submission Requirements
Board Certified - REQUIRED
NE license - Highly Preferred
SRS and SBRT - Highly Preferred
Case logs required at time of presentation - client will require before interviewing/offering provider
Please disclose any disciplinary actions taken on medical license (past or present) - REQUIRED
