Setting: Emergency Dept./Surgery
FTE: 1
Duties: Cover Urology call - get handoff from Urologists if appropriate
Required Procedures: General Urology
EMR: EPIC
Support Staff: Surgery nurses and techs. on call
Deal breakers (do not present this): Not willing to learn EPIC
Travel Preferences:
Preferred Lodging: Candlewood Suites
Air Travel: Local, preferred but not required
Credentialing Information:
Timeframe: 3-4 weeks minimum
Requirements: M.D. or D.O.; DEA; NE license; case logs from last 2 years
Submission Requirements
Board Certified/Board Eligible (Urology) - REQUIRED, NOTE AT NAME CLEAR.
Active NE license - HIGHLY PREFERRED
Clean Malpractice - HIGHLY PREFERRED
Availability of dates in the job description - REQUIRED, NOTE AT NAME CLEAR.
Case logs are required - REQUIRED, INCLUDE AT PRESENTATION.
Disclose if the provider has worked at any other CHI/Dignity facility - REQUIRED, NOTE AT NAME CLEAR.
COVID VACCINATED - REQUIRED, NOTE AT NAME CLEAR.
