open dates: Site is looking for one full time provider that can cover the dates below and dates starting week of Dec 15th for 3-6 months.

Board Certified/Board Eligible - Required
Active WA OR IMLC (must be in hand and uploaded) - Required
BLS - Required
Malpractice Accepted - Indicate limits you will consider: Highly preferred, candidates with self inquiry NPDB report will be placed with priority over those that dont present with it.
Availability at time of name clear - Required - must provide which of the open weeks the provider can cover.
CV- Must list work history and dates in MM/YY format with each facility listed out - Require
Specific dates and black out dates required for presentation - Require
Must list # of shifts provider is available to offer/month & if availability is shared between assignments - Require
