Prime Staffing is seeking a Nurse Practitioner Hospice for a locum tenens job in Escondido, California.

This would be in southwest Riverside county, so someone that lives in Temecula, Murietta, Lake Elsinore or Fallbrook would be preferred.n
nMon-Fri, and once a month on a Sunday and take a weekday offn8-5pmn
My Unity is the EMR nNP, Face to face hospice visits, working with the team and being a resource for home aids. 45-50 visits a month.
nMileage is 70 cents a mile
-Position: Nurse Practitioner providing home-based hospice and palliative care for patients with serious or life-limiting illnesses; conducts assessments, develops care plans, and manages symptoms.
-Patient Care: Performs home visits, goals-of-care discussions, advance care planning (POLST/advance directives), patient and family education, and coordinates care with the interdisciplinary team and community physicians.
-Schedule & Call: Rotating weekends required, occasional on-call coverage, and participation in 24/7 patient coverage when needed.
-Documentation: Documents all patient encounters in the EMR, participates in IDT meetings, quality initiatives, and collaborates closely with the Hospice & Palliative Care Medical Director.
-Requirements: California NP & RN licenses, DEA, NPI, MSN, AANP/ANCC certification, BLS, and 3+ years of hospice or palliative care experience. ACHPN certification required within one year (preferred at hire).
-Travel: Home-based position requiring local travel for patient visits; valid California driver's license and insurance required. Spanish bilingual is a plus.
-Ideal Candidate: Experienced hospice/palliative NP who is compassionate, independent, team-oriented, comfortable with end-of-life discussions, and passionate about delivering patient-centered care in the home.
